Surgery One is proud to be one of the first outpatient surgery centers in the country to track how our patients are doing. We want to be able to share with you how our surgeons and facilities are performing regarding your surgery! Our goal is to be as transparent as possible and provide you with a patient report card.
The Surgery One Network includes the finest Facilities and Physicians in their fields of expertise and how we demonstrate that is we strive for three outcomes.
- Less pain
- More function (the ability to do more activities in your life)
- Improved quality of life
The way we understand how well you, our patient, responds, is to measure your specific outcomes. In order for us to be able to offer this transparency we need to gather information from you regarding your surgery. We gather information from you pre- opand then again post- opat 6, 12 and 18 months.
Collection of data is vital to us to ensure our quality of care is in keeping with our high standards and that we are constantly monitoring how our surgeons and facilities are performing. The Surgery One's Network of facilities and physicians are carefully selected and a high standard of criteria is applied to all in our network.
With this in mind, we will ask you to respond to twenty three (23) questions covering the areas mentioned above:
- MTAP Walk and Climb (7 Questions): This questionnaire will ask you to respond to pictures representing activities you currently do from ‘able’ to ‘? Don’t know’.
- Patient Centered Outcome (3 Questions). This questionnaire asks you to list three (3) items you would like to be able to perform better (e.g. gardening, housework, recreational sports, lifting and carrying, getting in and out of car)
Quality of life (AQol) :
This questionnaire (12 questions) will ask you questions related to how you feel about the quality of your life. Some questions will be yes and no, others will be multiple choice.
In general, would you say your health is:
- very good
- good, fair
- or poor?
